09:00 | HOSPITAL READMISSIONS CRASH COURSE
Readmissions are undesirable for patients and providers and are a concern of health systems worldwide. This course aims to describe the importance of readmissions, how they can be identified and reduced. The discussion of both ongoing research and initiatives to reduce readmissions is encouraged.

Avoidable hospital admissions are adverse events for healthcare agents and patients, with great impact on health systems and for society. This course aims to describe case studies that evidence the importance, methods and initiatives to avoid hospital admissions using ambulatory care sensitive conditions.

The quality of the care provided to patients, in hospitals and elsewhere, is obviously an important concern and has been at the top of most health care systems’ agendas in the last decades. Portugal has had a set of interesting developments in this field in the past few years; the purpose of this session is to share and discuss these with international colleagues.
