Carme Hernández

Coordinator of the Integrated Care Unit, Hospital Clinic, ES


Carme Hernández, PhD, MSc, RN, FERS (Barcelona, 1961), is the coordinator of the Integrated Care (IC) Unit under the Medical and Nursing Administration Council. Since 2013 she has been a member of the Chronic program at the Hospital Clinic, being responsible for all the transitional Care Models. Dr. Hernández graduated from the Nursing School of the University of Barcelona (UB) in 1982 and worked in critical patient care units for 10 years in the Hospital Clinic. She has published more than 40 articles, reviews and editorials, over 15 book chapters and guidelines, and she has participated in several European projects. Currently she is a collaborator of the H2020 projects H2020 Selfie and Connecare,, and the regional project RIS3CAT Nextcare. Dr. Hernández actively participates in medical and scientific societies. She was Chair of the Nursing Group of the European Respiratory Society until 2006, a member of the European Lung Foundation and she was elected Fellow of the ERS (FERS) in 2015. In the Spanish Society of Pneumology and Thoracic Surgery (SEPAR) she was Chair of the Nursing and Physiotherapy Group until 2002 and she is a currently member of the Integrated Research Program of COPD “PII EPOC”, and co-chair of Separ Pacientes.
Furthermore, she has been a member of the COPD Strategy Committee of the Spanish Ministry of Health since 2008. In the American Thoracic Society (ATS) Nursing Assembly, she served on the Education and Program Planning Committees and is currently co-chair of the project on International Respiratory Research CARES (Critical care, Adult Respiratory Health, and Sleep). In 2005, she received the ATS "Nurse Competence in Aging Award", given for her outstanding commitment to care for older adults through research and practice. In 2005 Dr. Hernández received a two-year intramural grant from the director of the Hospital Clinic of Barcelona for the evaluation of chronic patients in a tertiary care hospital. Since 2009 Dr. Hernández has been a member of the “Pla Director de Malalties Respiratòries” of the Catalonian Ministry of Health. Dr Hernández received her PhD Degree in Medicine in 2015 (Cum Laude), and is a teacher of the Master of Respiratory Medicine (UB), Master of Nursing Science (University Jaume I), and Master of Chronicity and Dependence (UPF/UAB) programs at the University of Barcelona, among others. Her research lines focus on integrated chronic care models, patient stratification, fragile and complex patients, and programs for patient education and disease selfmanagement.
